I've been looking over some schematics from 50's designs and I've noticed that on some they do not have a cathode coupling cap (pin 3 or 8 on a 12 ax7) on their preamp tubes.

What is the purpose of the cap?
What effect does it have on the amp and to tone?
Why would it be (or not be) used?
How do you determine what size it should be?

Thanks!
